Job summary

Catholic Charities of Delaware, Otsego, & Schoharie Counties is seeking a STEPH Case Manager to support individuals experiencing homelessness by accepting referrals, completing assessments, and developing personalized service plans to connect clients with local resources and housing options.

The role involves regular contact, advocacy, transportation assistance, budgeting support, and collaboration with other departments and external agencies.

Qualifications

• Associates Degree in Human Services or related field is required.
• 1–3 years of experience working with a diverse population.
• Valid driver’s license with access to a reliable vehicle.

Responsibilities

• Accept and process referrals for individuals and families experiencing homelessness.
• Complete all required documentation and verifications for each household referred, including assessments and release forms; summarize findings and develop a service plan with goals and referrals.
• Determine frequency of contact with each household and ensure contact at least every two weeks.
• Provide support to overcome barriers to achieving goals, including financial assistance and transportation.
• Deliver direct services: advocacy, coordinating resources, referrals, transportation, budgeting, employment readiness, and housing-related financial assistance.
• Coordinate with Catholic Charities departments and external agencies to ensure comprehensive service.

Education
