Job summary

Mercy Home Health is seeking a registered nurse to deliver patient care using the nursing process, coordinate care with an interdisciplinary team, and drive safety and quality outcomes. The role includes initial and ongoing assessments, care planning, treatment implementation, and medication administration in line with state practice and Mercy Health policies.

You will collaborate across departments, provide patient teaching, advocate for patients, and participate in quality improvement

Qualifications

• Valid nursing license from the state of practice (required)
• BLS Basic Life Support - American Heart Association (preferred)

Responsibilities

• Conducts initial and ongoing patient assessment with the care team.
• Analyzes assessment data and creates a plan of care.
• Implements treatment and evaluates treatment effectiveness.
• Administers medications safely per state practice and policies.
• Serves as point of contact for care coordination across hospital departments.
• Participates in ongoing quality improvement and safety initiatives.
