Job summary

Brentwood Hospital in Shreveport, LA seeks a dynamic Patient Navigator to manage the step-down process and engage patients in outpatient services prior to discharge. You will collect data, report to departments and the CEO, and coordinate with PHP/IOP/ECT discussions within 24 hours of admission.

Qualifications

• Education: Associates degree in nursing or Master's degree in behavioral health related field required.
• Experience: Previous mental health experience preferred; must be competent and comfortable in dealing with all aspects of the mental health field.
• Licensure: RN or LPN, preferred.

The Patient Navigator manages the work-flow of the step-down process ensuring eligible patients are identified and engaged in discussion about outpatient services available at Brentwood Hospital prior to inpatient discharge. The step-down navigator will be responsible for data collection, communication and report generation to the involved departments and CEO.

ESSENTIAL JOB FUNCTIONS:

1. Manages the workflow of the step-down process by ensuring eligible patients are identified and engaged in PHP/IOP/ECT discussion as a next step in treatment prior to inpatient discharge.
