The Patient Navigator manages the work‑flow of the step‑down process ensuring eligible patients are identified and engaged in discussion about outpatient services available at Brentwood Hospital prior to inpatient discharge. The step-down navigator will be responsible for data collection, communication and report generation to the involved departments and CEO.
