Job summary

Carease Health is seeking a Medical Director to lead clinical care across a statewide network of Skilled Nursing Facilities in Ohio. You will oversee medical teams, mentor NPs, and drive evidence-based practices to improve outcomes.

Ideal candidates have geriatric or post-acute medicine expertise, active licenses, and SNF leadership experience. Travel across Ohio facilities is required to ensure consistent, high-quality care and cost-effective services.

Qualifications

• MD or DO from an accredited medical school.
• Board certification or board-eligibility in Internal Medicine, Family Medicine, Geriatric Medicine, or related specialty.
• Active, unrestricted medical license(s) in applicable state(s).
• Experience caring for older adults and medically complex patients.
• Fellowship training in Geriatric Medicine preferred.
• Prior SNF Medical Director experience.
• Experience developing clinical protocols, guidelines, or quality-improvement initiatives.
• Familiarity with CMS, Medicare, and regulatory requirements applicable to skilled nursing facilities.

Responsibilities

• Provide clinical leadership across a designated group of SNFs; conduct rounds with NPs and provide mentorship.
• Serve as a clinical resource for complex patients, diagnostic uncertainty, acute changes, and goals-of-care discussions.
• Promote proactive identification and management of residents at risk for deterioration or avoidable hospitalization.
• Collaborate with nursing, rehabilitation, and interdisciplinary teams to promote wellness and independence.
• Develop and implement evidence-based clinical guidelines, protocols, and care pathways.
• Monitor clinical quality metrics and lead initiatives to improve performance.
• Conduct case reviews and root-cause analyses to identify opportunities for improvement.
• Build relationships with facility and organizational leadership.
• Foster a culture of clinical excellence across national network.
• Deliver cost-effective, high-value care while reducing Medicare costs.
