Qualifications

• MD/DO from an accredited medical school.
• Board certification in Internal Medicine, Family Medicine, Geriatric Medicine, or related specialty.
• Active, unrestricted medical license(s).
• Experience caring for older adults and medically complex patients.
• Fellowship training in Geriatric Medicine preferred.
• Prior SNF Medical Director experience.
• Experience developing clinical protocols, guidelines, or quality‑improvement initiatives.
• Familiarity with CMS, Medicare, and regulatory requirements for SNFs.

Responsibilities

• Provide clinical leadership across a designated group of skilled nursing facilities.
• Conduct initial rounds with NPs and provide ongoing clinical mentorship and support.
• Serve as a clinical resource for complex patients and goals‑of‑care discussions.
• Promote proactive identification and management of residents at risk for deterioration or avoidable hospitalizations.
• Collaborate with nursing, rehabilitation, and interdisciplinary teams to promote wellness and independence.
• Develop and implement evidence‑based clinical guidelines, protocols, and care pathways.
• Monitor clinical quality metrics and lead initiatives to improve performance.
• Conduct case reviews and root‑cause analyses to identify opportunities for improvement.
• Build relationships with facility and organizational leadership.
• Foster a culture of clinical excellence across the national network.
• Deliver cost‑effective, high‑value care while reducing avoidable Medicare costs.
