Job description

Medical Doctor Skilled Nursing Facilities Statewide Travel

Company: Carease Health

Location: Indianapolis, IN

We are seeking exceptional physicians with a passion for geriatric, post-acute, and long-term care medicine to join our growing national medical provider services organization as Medical Directors.

Our organization provides proactive, high-quality care to residents in Skilled Nursing Facilities (SNFs), with a focus on promoting wellness, improving quality of life, identifying clinical issues early, and preventing avoidable hospitalizations.

Medical Directors will serve as regional clinical leaders, overseeing care across a portfolio of skilled nursing facilities within their geographic area. Working closely with nurse practitioners and facility teams, Medical Directors will elevate clinical decision-making, develop advanced clinical skills, and create a culture of excellence and accountability.

The goal is not simply to oversee nurse practitioners, but to develop highly capable clinicians who can independently provide excellent care while recognizing when physician collaboration or intervention is appropriate.

Medical Directors will partner closely with organizational and facility leadership, including administrators, DONs, case managers, and facility medical staff, to ensure consistent, evidence-based care and continuously improve clinical outcomes.

Key Responsibilities
  • Provide clinical leadership across a designated group of skilled nursing facilities.
  • Conduct initial rounds with NPs and provide ongoing clinical mentorship and support.
  • Serve as a clinical resource for complex patients, diagnostic uncertainty, acute changes in condition, and goals-of-care discussions.
  • Promote proactive identification and management of residents at risk for clinical deterioration or avoidable hospitalization.
  • Collaborate with nursing, rehabilitation, and interdisciplinary teams to promote wellness, mobility, and independence.
  • Develop and implement evidence-based clinical guidelines, protocols, and care pathways.
  • Monitor clinical quality metrics and outcomes and lead initiatives to improve performance.
  • Conduct case reviews and root-cause analyses to identify opportunities for improvement.
  • Build strong relationships with facility and organizational leadership.
  • Help foster a consistent culture of clinical excellence across the organization's national network.
  • Deliver cost-effective, high-value care while reducing avoidable Medicare costs.
Qualifications Required
  • MD or DO from an accredited medical school.
  • Board certified or board eligible in Internal Medicine, Family Medicine, Geriatric Medicine, or related specialty.
  • Active, unrestricted medical license(s) in applicable state(s).
  • Experience caring for older adults and medically complex patients.
  • Demonstrated interest in clinical education, mentorship, and leadership. Preferred
