Job Summary

Responsible for providing long-term care nursing and related services to residents consistent with comprehensive assessments and care plans. Delivers care that meets residents’ physical, mental, and psychosocial needs. Enables individuals to attain or maintain the highest practicable level of functioning. Maintains a homelike environment and protects resident rights. Assists residents in maintaining independence and control to the greatest extent possible.

Essential Functions
  • Provides direct care to residents to improve or maintain abilities according to the plan of care.
  • Assists residents with activities of daily living, including bathing, grooming, dressing, mobility, transferring, and feeding.
  • Transports residents to tubs or showers, adjusts water temperature, and ensures thorough washing, rinsing, and drying while maintaining privacy.
  • Selects and obtains clothing for residents, assists with dressing and undressing, and communicates clothing needs or repairs to the charge nurse.
  • Assists residents with toileting, provides clean bedpans or urinals, ensures proper hygiene, and records output as directed.
  • Provides catheter care and follows infection control procedures.
  • Assists with or provides range of motion exercises and weight-bearing support for ambulation.
  • Maintains a safe environment by removing obstacles, inspecting equipment, and reporting defects to the charge nurse.
  • Monitors resident progress toward treatment goals and reports responses to the charge nurse.
  • Performs other duties as assigned.
Education Requirement

High school diploma or equivalent and Vocational/technical training in a designated technical area.

Experience Requirement

0-1 years of experience in a healthcare, clinical setting, or a relevant area. An equivalent combination of education and experience may be considered. All experience must be paid and in the same related field. Part‑time and PRN experience will be prorated based on hours worked per week. Volunteer work and internships for academic credit are not counted.

Certifications & Licensures

Kentucky State Registered Nurse Assistance (SRNA) certificate.
