Job summary

Southwestern And Affiliates in Evansville, IN is seeking a full-time clinician to provide school and community-based case management and skills development for children and families. You will coordinate services with schools, DCS, Juvenile Probation and therapists to support patient progress.

Requirements include a Bachelor's degree in a mental health or human services field, at least two years of experience with long-term psychiatric patients, 21+ years old, and a valid Indiana driver's license

Responsibilities

• Maintain documentation and submit reports according to established time frames.

WHAT IS THIS POSITION RESPONSIBLE FOR?

• Provide school and community-based case management, skills training/development, and other services to children and their families.
• Work with children and their families referred from the school as well as the Department of Child Services, Juvenile Probation, System of Care, Southwestern assessment team and therapists, and other referral sources.
• Maintain appropriate records of services to reflect patient progress and document treatment plans and goals.
• Maintain documentation and submit reports according to established time frames.
• Communicate professionally, make referrals and coordinate services with community agencies.

Qualifications: WHAT'S REQUIRED FOR THIS POSITION?

• Must hold a Bachelor's degree in a mental health or humans services related field with a minimum of two years of experience working with long-term psychiatric patients.
• Must be at least 21 years of age, maintain a valid Indiana driver's license and valid car insurance.
• Must obtain and maintain crisis management and verbal de-escalation training.
