Job summary

YAI - Seeing beyond disability seeks a NYSTART Coordinator for Region 2 Central in East Syracuse. The role manages a caseload of individuals served by START, performing evaluations, intake assessments, and cross-system coordination through home or telehealth visits.

The position involves referrals, crisis planning, and regular contact with the clinical team, ensuring timely documentation and training compliance across regions. Travel within the state is required.

Qualifications

• Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field.
• Ability to provide on-call crisis coverage as needed.
• Strong interpersonal and problem-solving skills.

Responsibilities

• Maintains an active caseload of 20–25 individuals referred for START Services.
• Conducts regular home and program visits via in-person or telehealth.
• Ensures coordination of support meetings and crisis plans with clinical leadership.
• Completes intake assessments and referrals to the clinical team as needed.
• Enter data into START Information Reporting System and related systems.
