Advocate Aurora Health is seeking a Registered Nurse (RN) for Home Health Case Management in Whitefish Bay, Wisconsin.

Job Description & Requirements
  • Specialty: Case Management

  • Duration: Ongoing
  • 40 hours per week
  • Shift: Days
  • Employment Type: Staff
Major Responsibilities:
  • Manage caseloads of patients in specialty areas.
  • Conduct initial assessments and establish care plans.
  • Provide direct patient care, adjust nursing care as needed, and evaluate outcomes.
  • Communicate with physicians and other team members to update and coordinate care.
  • Complete accurate documentation including visit assessments, OASIS, 485, and verbal orders.
  • Participate in discharge planning and process improvement activities.
  • Maintain accurate clinical records per guidelines.
  • Provide age-appropriate care, demonstrating knowledge of growth and development principles.
Licensure, Registration, and Certification:
  • State-issued RN license
  • Basic Life Support (BLS) certification (must be obtained within 6 months if not already held)
  • Valid driver’s license and reliable transportation
Education:
  • Associate's Degree in Nursing
Experience:
  • Typically 3 years of clinical nursing experience
Skills & Abilities:
  • Excellent communication and interpersonal skills
  • Ability to work well with physicians and professionals
  • Proficiency in clinical skills and judgment
  • Ability to operate necessary equipment
  • Ability to assess patient data and interpret information
  • Respect for diversity and cultural sensitivity
  • Valid driver’s license and insured vehicle
Physical Requirements & Working Conditions:
  • Manual dexterity and physical stamina to perform duties
  • Ability to move in confined spaces and lift specified weights
  • Ability to work safely with diverse patient conditions
  • Ability to sit, stand, walk, climb stairs, and perform transfers
  • Visual acuity for reading gauges and instructions
  • Exposure to weather, chemicals, and communicable diseases
