No Weekends

$5/hr night differential

The ACE unit focuses on preventing loss of independence in older patients during hospitalization through a structured, interdisciplinary approach.

Major Responsibilities:

• Assess patient needs, plan, implement, and evaluate care, involving patients and families.
• Recognize and respond to significant changes in patient condition, coordinating care accordingly.
• Implement strategies to reduce risks and improve safety.
• Coordinate discharge planning with an interdisciplinary team.
• Evaluate patient outcomes and support quality improvement initiatives.
• Communicate effectively, promote collaboration, and take leadership roles.
• Ensure safety and report safety events timely.
• Participate in professional development and adhere to age-specific care standards.

Licensure, Registration, and Certifications Required:

• RN license issued by the state of practice
• Basic Life Support (BLS) certification from AHA

Education Required:

• Completion of an accredited Nursing program

Experience:

• New Graduates welcome!

Knowledge, Skills & Abilities:

• Holistic patient care, clinical judgment, computer proficiency, communication, teamwork, decision-making, organization, and time management skills.

Physical Requirements and Working Conditions:

• Ability to sit, stand, lift, and perform physical tasks as described, including lifting up to 50 lbs, and exposure to potentially hazardous materials with proper protective equipment.
