Qualifications

Staff - Registered Nurse (RN) - Home Health Case Management - $37-56 per hour

Advocate Aurora Health is seeking a Registered Nurse (RN) in Home Health Case Management for a nursing position in Flossmoor, Illinois.

Job Description & Requirements

Location: Far Southeast City/Suburbs, focusing on Calumet City, Lynwood, Dolton, Glenwood, Lansing, Posen, South Holland, Thornton, Burnham, Riverdale

Hours: Days; must be able to work one summer and one winter holiday. Must be able to rotate on an on-call schedule.

This position is eligible for a up to $10,000 sign-on bonus, payable when employed in the position at the time of payout. Internal team members are not eligible for this bonus.

Major Responsibilities:

  • Nurse/Patient Relationship: Develops and maintains therapeutic relationships, demonstrates adaptive communication, provides individualized care, involves patients and families in planning, and adopts a holistic approach.
  • Assuring/Improving Quality Care: Ensures quality, cost-effective care, mobilizes resources, adheres to standards, participates in quality initiatives, and promotes patient safety.
  • Patient/Staff Education: Meets educational needs of patients and families, utilizes resources, and fosters educational growth among staff.
  • Nursing Process: Uses nursing problem-solving, recognizes clinical changes, makes clinical decisions, prioritizes care, manages referrals and discharge planning, and applies ethical principles.
  • Leadership/Collaboration/Professional Development: Collaborates effectively, demonstrates leadership, participates in team activities, maintains current knowledge, and earns required certifications or degrees.

Licensure:

  • Registered Nurse (RN) license in Illinois
  • Valid driver’s license

Education/Experience:

  • Graduate of an accredited nursing program
  • Minimum of 10 continuing education credits annually
  • At least one year of medical-surgical experience

Knowledge, Skills & Abilities:

  • Strong communication and interpersonal skills
  • Good documentation and computer skills
  • Time management and organizational skills
  • Familiarity with patient care equipment
  • Current Illinois RN license and CPR certification

Physical Requirements and Working Conditions:

  • Ability to work flexible hours, including weekends, holidays, and on-call
  • Ability to handle multiple tasks and respond quickly to patient needs
  • Ability to lift up to 35 pounds without assistance; patient handling equipment used for heavier lifts
  • Must be willing to travel within the service area in various weather conditions
