This position offers up to a $10,000 sign-on bonus, subject to taxes, and requires working one summer and one winter holiday, with on-call responsibilities. The RN will provide home care services, coordinate medical needs, and establish relationships with patients and caregivers to support independence and prevent rehospitalization.

Responsibilities

• Assess, plan, implement, and evaluate nursing care in patients' homes.
• Develop therapeutic relationships with patients and families.
• Coordinate with the care team using point-of-care documentation.
• Participate in leadership and professional development activities.
• Assist with patient mobility and handling, including lifting up to 35 pounds without assistance and aiding in transfers up to 200 pounds.
• Travel within the service area, working in varied environments including private residences.

Qualifications

• Graduate from an accredited nursing program.
• One year of medical-surgical experience in an acute or subacute setting.
• Current Illinois RN license and CPR certification.
• Ability to earn 10 continuing education credits annually.
• Physical ability to perform required lifting and mobility tasks.
