Staff - Registered Nurse (RN) - Home Health Case Management - $37-56 per hour

Job Details

• Location: Southwest Suburbs, Illinois (Oak Lawn, Burbank, Justice, Bridgeview, Hickory Hills, Willow Springs, Chicago Ridge)
• Hours: Days, with weekend and holiday rotation
• Employment Type: Full-time, Staff
• Pay: $37-56 per hour, with up to a $10,000 sign-on bonus (taxable), must be employed at payout time
• Schedule: Must be able to work one summer and one winter holiday, and rotate on-call

Position Overview

The RN will provide home care services to the homebound population, establishing strong relationships with patients and caregivers to maximize patient potential. Responsibilities include coordinating medical needs, documenting care, and working with a multidisciplinary team.

Responsibilities

• Assess, plan, implement, and evaluate nursing care at patients' homes
• Develop holistic care goals
• Maintain therapeutic relationships with patients and families

Qualifications

• Graduate from an accredited nursing program
• Current Illinois RN license and CPR certification
• At least 1 year of medical-surgical experience in an acute or subacute setting
• Ability to lift up to 35 pounds without assistance; assist with patient transfers up to 200 pounds
• Good communication, documentation, and computer skills
• Ability to work flexible hours, including weekends, holidays, and on-call shifts

Physical and Work Environment

• Work involves travel to private residences, some not accessible to individuals with disabilities
• Exposure to potentially hazardous conditions and infectious materials
• Must be able to respond quickly and work in varying weather and social conditions
