This provider plays a central role within the Interdisciplinary Team (IDT), ensuring high-quality, patient-centered, and regulatory-compliant care. Care delivery spans multiple settings, including the PACE center, participants’ homes, virtual consultations, and skilled nursing facilities (SNFs). The provider is responsible for managing a defined panel of participants, promoting wellness, preventing illness, and coordinating care with specialists and IDT members.**Minimum Requirements*** Primary Care Physicians who are Board-certified and eligible in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics.* Physician with an active unrestricted CA license to practice medicine by the Medical Board of California.* Previous experience with computer-based systems required; Electronic Medical Records experience preferred.* Current DEA license required.* A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.**Compensation**$237,078.40 - $296,337.60 annually**Compensation Disclaimer**Actual salary offers are considered by various factors, including budget, experience, skills, education, licensure and certifications, and other business considerations.
