Job summary

Polaris Pharmacy Services in Norfolk, VA is seeking a Staff Pharmacist to support day-to-day dispensing for long-term, post-acute, and specialty pharmacy services.

You will verify labels, manage inventories, and ensure compliance with state and federal guidelines while delivering compassionate patient care.

Qualifications

  • Active pharmacist license in the state of practice.
  • Bachelor’s degree or Doctor of Pharmacy from an accredited school.
  • Minimum of two years’ recent experience in an acute-care setting or one year as a production pharmacist preferred.

  • Maintain resident database with current medical history and medications.
  • Assist in ordering, handling, receiving, documenting, and storage of controlled substances.

The Staff Pharmacist must adhere to the company’s quality and customer service standards, policies and procedures, as well as the State and Federal guidelines.Duties/Responsibilities:Verification for completeness and accuracy of all new and refill labels and orders. Check for content and accuracy of all medication packaging and labeling including emergency drug kits and IV medicationsMaintain resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including OTC and home remedies)Assist in the proper ordering, handling, receiving, documentation, and storage of all controlled substances; assist in maintaining a perpetual inventory for schedule II drugsWork with appropriate pharmacy technician in reconciling Controlled Substances perpetual inventory as required by management for compliance with state and federal regulationsProvide clinical expertise by responding to drug-related questions and concerns initiated by internal staff and external customersDocument all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical recordPreparation and/or supervision of all sterile IV drug products or solutionsProvide evening, weekend, and holiday coverage during rotation and as required by managementFollow all applicable government regulations including HIPAAOther duties as assigned; Job duties may vary depending on business needsResponsible for the flow of work and appropriate outcomesQUALIFICATIONS/COMPETENCIES:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
