Qualifications

Position Summary

We are seeking a dedicated and experienced Home Infusion Pharmacist to join our team. The ideal candidate will be responsible for providing safe and effective drug therapy to home care patients, collaborating with physicians, nurses, and other healthcare professionals. This role includes clinical decision-making, sterile compounding, patient monitoring, and ensuring compliance with USP cleanroom standards.

Minimum Qualifications

  • Education: Bachelor of Science in Pharmacy required. Doctor of Pharmacy (PharmD) or Master of Science in Pharmacy preferred.
  • Licensure: Active New York State Pharmacist license required.
  • Experience:
    • Minimum of 2 years recent experience in an acute-care setting, or
    • 1 year recent experience in home infusion therapy (preferred).
  • Strong knowledge and experience in intravenous admixture services and IV therapy.
  • Proven ability to work collaboratively with interdisciplinary teams.
  • Excellent problem-solving, supervisory, organizational, and time management skills.
  • Dependable attendance and job performance.
  • Membership in professional societies desirable.
  • Outstanding written and verbal communication skills.
Key Responsibilities

Pharmacy Support & Clinical Services

  • Make sound clinical and business decisions related to patient care.
  • Function independently while documenting all patient and departmental activities.
  • Collaborate across departments (e.g., during rounds or case conferences).
  • Supervise, support, and educate technical staff.
  • Perform in-services as requested or required.
  • Evaluate and monitor drug therapy in relation to patients' comprehensive needs.

Patient Care & Therapy Management

  • Participate in daily rounds and obtain/verify infusion orders from physicians.
  • Assess safety and appropriateness of drug therapy for each patient.
  • Evaluate and recommend changes to TPN (Total Parenteral Nutrition) therapy.
  • Maintain accurate and updated clinical records, care plans, and progress notes.
  • Contact patients to coordinate delivery and ensure quality care.

Sterile Compounding / USP Standards Compliance

Follow USP and USP cleanroom protocols:
  • Perform appropriate garbing, hand hygiene, and sterile technique.
  • Maintain aseptic conditions while compounding.
  • Monitor cleanroom standards and ensure compliance with SOPs.
  • Handle, label, and store compounded preparations appropriately.

Operational & Departmental Improvement

  • Troubleshoot operational issues and assist in process improvements.
  • Mentor and train pharmacy staff to enhance performance.
  • Contribute to departmental policy compliance and efficiency initiatives.
  • Identify and implement clinical changes to improve patient outcomes.

Teamwork & Professional Development

  • Demonstrate initiative and accountability.
  • Stay current with new products, services, and safety procedures.
  • Engage positively with co-workers, prescribers, and patients.
  • Respond to policy updates and internal communications promptly.
