Job summary

Great Lakes Bay Health Centers is seeking a dedicated Registered Nurse to provide direct patient care, triage, and ongoing education on chronic conditions and medications. The role emphasizes coordination with a multi-disciplinary team and performing clinical assessments to support provider visits.

The ideal candidate will have a BSN or equivalent nursing education, Michigan RN licensure, and strong communication skills to engage diverse patient populations.

Qualifications

• Graduate from an accredited professional nursing program (BSN preferred).
• Current RN license to practice in Michigan; Basic Life Support (BLS) certification.
• Excellent organizational, written, and verbal communication skills; able to engage diverse populations.

Responsibilities

• Direct patient care duties: review and adjust treatment plans, triage by phone or walk-in, and monitor patient conditions.
• Provide ongoing patient and family education on chronic conditions, disease processes, and medication management.
• Perform clinical assessments and diagnostic procedures per nursing protocols and provider directions; contribute to care planning and coordination.
