Job summary

Vitality Hospice is seeking a part-time Spiritual Care Coordinator (Chaplain) to provide patient-centered spiritual care to hospice patients, families, and caregivers. You will work with the Interdisciplinary Team to ensure culturally sensitive care aligned with hospice philosophy and regulatory standards.

The role involves on-the-road assignments, evening/weekend on-call rotation, and collaboration with clergy and community spiritual resources while supporting bereavement needs and staff

Qualifications

• Strong communication, planning, and organizational skills.
• Experience working with death and dying.
• Ability to work in an interdisciplinary team.
• Comfort in interfaith, multicultural environments.
• Knowledge of hospice philosophy and regulatory standards.

Responsibilities

• Assess spiritual needs and provide supportive spiritual care to patients and families.
• Develop and document individualized spiritual care plans.
• Collaborate with the Interdisciplinary Team to coordinate holistic care.
• Participate in IDT meetings and care plan reviews.
• Provide consultation, education, and support to staff on spiritual care.
