Job summary

Aveanna Healthcare is hiring a Speech Therapist for Part-Time PRN in the Mauston, Reedsburg, Dells, Baraboo, and Sauk area of Wisconsin. You will develop and implement home health therapy plans, coordinating with physicians and the Clinical Manager to ensure services align with licensing and agency policies.

Responsibilities include assessing client needs, implementing therapy plans, and maintaining comprehensive medical documentation while guiding clients and families toward optimal

Qualifications

• Current Speech Therapy license in the state of application.
• CPR certification required.
• Valid driver’s license and automobile insurance.

Responsibilities

• Formulate or modify agency policies, procedures, and practices as needed
• Ensure timely documentation and transfer of medical data to physicians and caregivers
• Maintain accurate client medical data
• Instruct clients and caregivers to maintain or improve functioning
• Investigate and address client or employee concerns and complaints
• Review client data to assess therapy effectiveness

Now Hiring: Speech Therapist, Home Health Part Time PRN

Schedule: Monday through Friday

Territory: Mauston, Reedsburg, Dells, Baraboo, and Sauk

The Speech Therapist Part Time PRN will develop and implement care plans for their clients that coordinate the clients’ home care services and follow physicians’ orders. Ensuring that services are delivered according to licensing guidelines, professional medical standards, and agency policies and procedures.

What You'll Do

• Assess and develop a program for therapies that meet individual client needs
• Implement the client’s therapy plan
• Coordinate therapy modifications to the client’s plan of care with the Clinical Manager and client’s physician
• Work withthe Branch Manager and Clinical Manager to formulate or modify agency policies, procedures, and practices as needed
• Ensure effective coordination of client services through timely completion of required documentation and transfer of pertinent medical data to the client’s physician, Clinical Manager, and other caregivers
• Maintain accurate and comprehensive client medical data
• Instruct clients, family members, and primary caregivers as needed to ensure the maintenance of or acquisition of optimal functioning level for each client
• Investigate and take appropriate actions on client or employee concerns and complaints
• Routinely review pertinent client medical data to determine the effectiveness of therapy services in reaching maximum rehab potential
