Job summary

Franklin Restorative Care Center in Minnesota is seeking a Speech Therapist (SLP) to evaluate, diagnose, and treat communication, cognitive, and swallowing disorders. The role focuses on helping residents improve speech, language, voice, cognition, and safe swallowing to maximize independence and quality of life.

You will develop individualized treatment plans, document progress, educate residents and families, and collaborate with physicians, nursing, and other therapy disciplines to ensure

Qualifications

• Current and active SLP license in Minnesota (or eligibility).
• Master’s degree in Speech-Language Pathology.
• CCC-SLP from ASHA preferred.
• Strong assessment, treatment, and documentation skills.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.

Responsibilities

• Evaluate residents/patients for communication, cognitive, and swallowing needs.
• Develop and implement individualized treatment plans based on assessments and physician orders.
• Provide therapy services to improve speech, language, voice, fluency, memory, problem-solving, and swallowing safety.
• Document evaluations, progress notes, and discharge summaries in compliance with professional, state, and federal standards.
• Educate residents, families, and staff on communication strategies, swallowing precautions, and home programs.
• Collaborate with physicians, nursing, and other therapy disciplines to ensure coordinated care.
• Participate in interdisciplinary care plan meetings and quality improvement programs.
• Maintain professional licensure and adherence to ethical practice standards.

Key Responsibilities:

• Evaluate residents/patients for communication, cognitive, and swallowing needs

• Develop and implement individualized treatment plans based on assessments and physician orders

• Provide therapy services to improve speech, language, voice, fluency, memory, problem-solving, and swallowing safety

• Document evaluations, progress notes, and discharge summaries in compliance with professional, state, and federal standards
