Interim HealthCare of Rochester is seeking a Home Health Speech-Language Pathologist (SLP) to provide patient-centered care in the home. You’ll evaluate and treat communication, swallowing, and cognitive issues, collaborating with a multi-disciplinary team to meet established goals.

This PRN position offers flexible assignments, mileage reimbursement, and opportunities for CEUs while delivering high-quality, personalized therapy in patients’ homes across Rochester, MN 55901 and surrounding areas.

Qualifikationen

• Master's or Doctoral degree in Speech-Language Pathology and active SLP license in Minnesota.
• Minimum 1 year of speech therapy experience, ideally in home healthcare.
• CPR certification required.
• Knowledge of state and federal home health regulations.

Aufgaben

• Provide home-based speech therapy to patients with communication, swallowing or cognitive difficulties.
• Work with a home health team (RN, LPN, CNA, HHA, PT, OT) on patient plans of care.
• Observe and evaluate patient communication, swallowing and processing abilities.
• Conduct patient assessments, document progress and ensure goals are met.
• Assist with mouth, breathing and swallowing exercises, fluency, articulation and reading.
• Educate patient and family on plan of care, therapy goals and safety practices.

As a Home Health Speech-Language Pathologist, here’s a big-picture view of what you’ll do:

• Provide home-based speech therapy to patients having communication, swallowing or cognitive difficulties due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and OT, focused on the patient’s plan of care and established goals
• Observe and evaluate a patient’s ability to communicate, swallow and process information
• Conduct patient assessments, document progress and ensure patient goals are met
• Assist with mouth, breathing and swallowing exercises, fluency, articulation and reading
• Educate patient and family on plan of care, therapy goals, safety practices and self-care

A few must-haves for Home Health Speech-Language Pathologists:

• Master's or Doctoral degree in Speech-Language Pathology and active SLP license in Minnesota
• Minimum of 1 yearof speech therapy experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgement, critical thinking skills, strong communicator, empathy and compassion
