Interim HealthCare of Rochester is hiring a Home Health Speech-Language Pathologist to provide home-based therapy for patients with communication, swallowing, or cognitive challenges.

The role requires a Master’s degree in Speech-Language Pathology, Minnesota SLP license, CPR, and at least 1 year of home health experience. You’ll collaborate with a multidisciplinary team, document progress, educate families, and support patient goals in a flexible PRN schedule.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology and active SLP license in Minnesota.
• Minimum of 1 year of speech therapy experience, ideally in home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Good clinical judgement, strong communication, empathy and compassion.

Responsibilities

• Provide home-based speech therapy to patients with communication, swallowing or cognitive difficulties.
• Work as part of a home health team focused on patient plan of care and goals.
• Observe and evaluate a patient’s communication, swallowing and processing abilities.
• Conduct assessments, document progress and ensure goals are met.
• Educate patients and families on plan of care, therapy goals, safety practices and self-care.
