Job summary

Interim HealthCare is seeking a Home Health Speech-Language Pathologist (SLP) in Rochester, MN. This PRN role focuses on delivering home-based speech therapy to patients with communication, swallowing, or cognitive challenges, within a team-driven care plan.

Requirements include a Master’s/Doctoral degree in Speech-Language Pathology, an active Minnesota SLP license, CPR certification, and at least 1 year of experience, preferably in home health.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology is required.
• Active SLP license in Minnesota is required.
• Minimum of 1 year speech therapy experience, ideally in home healthcare.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Good clinical judgement, critical thinking, strong communication, empathy and compassion.

Responsibilities

• Provide home-based speech therapy to patients with communication, swallowing or cognitive difficulties.
• Collaborate with a home health team (RN, LPN, CNA, HHA, PT, OT) to follow the plan of care.
• Observe and evaluate a patient’s communication, swallowing and processing abilities.
• Conduct patient assessments, document progress and ensure goals are met.
• Assist with mouth, breathing and swallowing exercises, fluency, articulation and reading.
• Educate patient and family on plan of care, therapy goals, safety practices and self-care.
