Job summary

Breezy Hills Rehabilitation and Care Center in Lakeland, FL is seeking a compassionate Speech-Language Pathologist to join our rehabilitation team on a PRN basis. You will provide individualized speech, language, cognitive, and swallowing therapy to residents in a skilled nursing and rehabilitation setting.

Responsibilities include evaluating residents, creating treatment plans, and collaborating with an interdisciplinary team to improve communication and swallowing.

Qualifications

• Graduate degree in Speech-Language Pathology from an accredited program.
• Current or eligible Florida SLP license.
• Eligible for or holding ASHA CCC-SLP certification.
• Experience in SNF/rehabilitation/long-term care preferred.

Responsibilities

• Evaluate residents and develop individualized speech, language, cognition, and swallowing treatment plans.
• Provide therapy to improve communication, cognition, swallowing, and functional independence.
• Treat residents with speech, language, voice, and dysphagia disorders.
• Develop and implement therapeutic programs based on residents’ needs and goals.
• Recommend and train residents in adaptive communication devices when appropriate.
• Monitor progress and modify treatment plans as needed.
• Collaborate with nursing, therapy, dietary, and interdisciplinary team members.
• Participate in care plan meetings and conferences.
• Educate residents, families, and staff on communication strategies.
• Supervise supportive personnel in accordance with regulations.
• Maintain accurate clinical documentation and comply with standards.

About The Position

Breezy Hills Rehabilitation and Care Center is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis. The Speech Therapist will provide individualized speech, language, cognitive, and swallowing therapy to residents in a skilled nursing and rehabilitation setting.

Responsibilities

• Evaluate residents and develop individualized speech, language, cognitive, and swallowing treatment plans.
• Provide therapy to improve communication, cognition, swallowing, and functional independence.
• Treat residents with speech, language, voice, and dysphagia/swallowing disorders.
• Develop and implement therapeutic programs based on each resident's needs, abilities, and goals.
• Recommend and train residents in the use of adaptive and augmentative communication devices when appropriate.
• Monitor resident progress and modify treatment plans as needed.
• Collaborate with nursing, therapy, dietary, and other interdisciplinary team members.
• Participate in care plan meetings, staffing, and resident care conferences.
• Educate residents, families, and facility staff on communication and swallowing strategies.
• Supervise supportive personnel in accordance with professional and state regulations.
• Maintain accurate clinical documentation and comply with facility, state, and professional standards.

Requirements

• Graduate degree from an accredited Speech-Language Pathology program.
• Current or eligible Florida Speech-Language Pathology license.
• Eligible for or holding ASHA Certificate of Clinical Competence (CCC-SLP).
• Experience in SNF, skilled nursing, long-term care, rehabilitation, or healthcare preferred.
• Strong communication, clinical, organizational, and interpersonal skills.
