We are hiring for a Speech Therapist, PRN!

Responsibilities

The Speech Therapist in Home Health assesses and evaluates patient needs related to speech and language disorders, communication, swallowing, and cognitive impairments. Responsibilities include:

• Providing services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
• Evaluating patients' function levels and speech therapy needs using standardized tools, and consulting with physicians to develop the therapy plan.
• Treating speech production, fluency, language, cognition, voice, resonance, swallowing, and auditory habilitation/rehabilitation based on evidence-based practices.
• Monitoring and reporting patient responses and condition changes to the healthcare team.
• Educating patients, families, caregivers, and team members in therapy areas.
• Visiting patients across the service area and nearby agencies as needed.
• Completing progress notes after each visit and submitting them per policy.
• Developing and updating the plan of care within state guidelines, and reporting outcomes within 24 hours of evaluation.
• Participating in staff meetings, committees, and providing in-service education as required.
• Preparing discharge summaries or condition reports every 60 days.
• Maintaining up-to-date clinical knowledge and licensure.
• Participating in weekend rotations to provide services as needed.

Education and Experience

License Requirements

• Current license to practice Speech Therapy in Florida.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle.
