Responsibilities

The Speech Therapist in Home Health assesses and evaluates patient needs related to speech and language disorders, communication, swallowing, and cognitive impairments. Based on this, they develop and implement treatment plans, perform interventions, and monitor patient progress.

• Provide services within the scope of practice defined by state laws, in accordance with the care plan, and in coordination with the healthcare team.
• Evaluate patients' functional levels and speech therapy needs, and consult with physicians to develop therapy plans.
• Treat speech production, fluency, language, cognition, voice, resonance, swallowing, and auditory rehabilitation using evidence-based practices.
• Document patient responses and communicate changes to the healthcare team.
• Educate patients, families, and caregivers about therapy and patient care strategies.
• Visit patients across the service area, including nearby LHC agencies as needed.
• Complete and submit progress notes and evaluations as per policy and guidelines.
• Participate in staff meetings, committees, and provide in-service education as required.
• Prepare discharge summaries and periodic reports on patient progress.
• Maintain current licensure and participate in ongoing education to stay updated with standards of care.
• Participate in weekend rotations to provide services as needed.

Education and Experience

License Requirements

• Current license to practice Speech Therapy in Florida
• CPR certification
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle
