Responsibilities

The Speech Therapist in Home Health assesses and evaluates patient needs related to speech and language disorders, communication, swallowing, and cognitive impairments. They develop treatment plans, perform interventions, and evaluate patient progress.

• Provide services within the scope of practice as defined by state laws and in coordination with the healthcare team.
• Evaluate patients' functional levels and speech therapy needs using standardized tools; consult with physicians on treatment plans.
• Treat speech production, fluency, language, cognition, voice, resonance, swallowing, and auditory habilitation/rehabilitation based on evidence-based practices.
• Observe, record, and report patient responses and condition changes to supervisors and physicians.
• Instruct patients, families, caregivers, and healthcare team members within the scope of speech-language pathology.
• Visit patients across the service area and neighboring agencies as needed.
• Complete and submit progress notes and evaluations according to policy, reporting outcomes and treatment plans promptly.
• Participate in staff meetings, committees, and provide in-service education as requested.
• Prepare discharge summaries and periodic reports on patient conditions.
• Adhere to LHC Therapy Standards of Practice and maintain up-to-date clinical knowledge and licensure.
• Participate in weekend rotations as needed.

Education and Experience

License Requirements

• Current license in Speech Therapy in Florida
• CPR certification
• Valid driver's license, vehicle insurance, and access to reliable transportation
