Job summary

North Country Healthcare (NCH) seeks a Speech Therapist to evaluate and treat expressive, receptive, cognitive, and feeding/swallowing disorders across inpatient, outpatient, and home health patients.

The role requires a Master’s in SLP with CCC-SLP eligibility, NH licensure, and a driver’s license. Must work within a high reliability organization and collaborate with an interdisciplinary team.

Qualifications

• Master’s Degree in Speech/Language Pathology from an ASHA-accredited program.
• CCC-SLP certification or eligibility pending.
• NH SLP licensure and valid driver’s license.
• BLS or Heartsaver CPR AED with regular renewals, and grace period after expiration.

Responsibilities

• Conduct initial evaluations with objective goals for each referred patient.
• Develop written treatment plans in consultation with the referring provider.
• Maintain current progress notes in the patient’s record.
• Ensure equipment is safe and report any unsafe equipment.
• Maintain speech therapy supplies at reasonable par levels.
• Develop and update policies and procedures for Speech Therapy practice.
• Attend Rehab Services Manager designated meetings.
• Provide care across inpatient, outpatient and home health settings.
