Harbor Point Skilled Nursing seeks a licensed Speech-Language Pathologist to provide PRN speech therapy to residents and short-term patients. You will develop and implement plans of care, screen patients, and collaborate with an interdisciplinary team to meet goals.

This role offers CEU and licensure reimbursements and emphasizes safe, patient-centered care in a family-like atmosphere. New grads are welcome to apply, with in-house therapy support and updated equipment to optimize patient time.

Responsabilités

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients.
• Assist as needed with patient screens for Speech Therapy.
• Communicate progress and any barriers toward SLP goals with interdisciplinary team members.
• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.
• Assist with maintaining rehab gym equipment and treatment area in a safe manner, using infection prevention methods.
• Provide and document patient, family, and caregiver training regarding plan of care and follow-up recommendations.
• Maintain productivity standards as expected by Rehab Director or Administrator.

This position is for a skilled nursing facility with both short-term and long-term patients. Harbor Point Skilled Nursing has an in-house therapy team with smaller daily case loads, all new equipment, and an emphasis on optimizing time with patients. PRN!!!!!!!

Speech Therapist Job Responsibilities:

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients.
• Assist as needed with patient screens for Speech Therapy.
• Communicate progress and any barriers toward SLP goals with interdisciplinary team members.
• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each speech therapy patient’s specific plan of care and recommendations for follow-up.
• Maintain productivity standards as expected by the Director of Rehab or Administrator.

Schedule: PRN

Education, Experience, and Licensing Requirements:

• Current Texas licensure as an SLP is required

Salary:

From $55.00 per hour
