Job summary

Creative Solutions in Healthcare, Inc. is seeking a Speech Language Pathologist to join our in-house team serving residents with person-centered care.

This full-time or PRN role focuses on evaluating swallowing and cognition, developing individualized treatment plans, and enhancing communication. You will work in a supportive setting with opportunities for education of residents and caregivers, collaboration with care staff, and timely, accurate documentation while ensuring compliance with

Qualifications

  • Active/Valid Texas license in Speech-Language Pathology
  • In good standing with regulatory agencies and licensing boards
  • SNF/Long term care experience preferred
  • Current knowledge of treatment practices
  • Knowledge of Net Health Electronic Documentation System preferred
  • Ability to manage patients with different types of personalities

Responsibilities

  • Evaluate residents' conditions for swallowing or cognitive deficits
  • Help residents achieve goals to improve cognition or swallowing
  • Education for residents and caregivers
  • Provide skilled, medically necessary speech and language therapy assessments and develop/implement individualized treatment plans to increase independence
  • Provide AAC systems for individuals with severe expressive/language disorders
  • Provide training and education to clients, families, caregivers, and other professionals
  • Coordinate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physician
  • Demonstrate competent technical skills according to current speech therapy standards of practice
  • Communicate changes in resident’s condition
  • Provide accurate and timely billing records and documentation
  • Respond to family requests and concerns promptly
  • Coordinate training with Restorative Nursing and other care staff
  • Assure resident abuse or neglect does not occur
  • Observe confidentiality and safeguard patient information
  • Complies with federal, state, and local legal and professional requirements

Responsibilities
  • Evaluate Residents conditions for swallowing or cognitive deficits.
  • Help Residents in their goals to improve Cognition or Swallowing
  • Education for Residents and Caregivers
  • Provide skilled, medically necessary speech and language therapy assessments as well as develop and implement individualized treatment plans to increase client’s independence and functioning.
  • Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders.
  • Provide training and education to clients, family, caregivers, and other professionals as a subject matter expert.
  • Coordinate and collaborate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physician.
