Job summary

Harbor Point Skilled Nursing in Texas is seeking a Speech-Language Pathologist (SLP) for PRN coverage across short-term and long-term residents. You will develop and implement plans of care, screen patients, and document progress within an interdisciplinary team.

Responsibilities

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients
• Assist as needed with patient screens for Speech Therapy
• Communicate progress and barriers toward SLP goals with interdisciplinary team
• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements
• Maintain rehab gym equipment and treatment area safely with infection prevention methods
• Provide and document patient, family, and caregiver training regarding plan of care and follow-up
• Maintain productivity standards as directed

Job description

This position is for a skilled nursing facility with both short-term and long-term patients. Harbor Point Skilled Nursing has an in-house therapy team with smaller daily case loads, all new equipment, and an emphasis on optimizing time with patients. PRN!!!!!!!

Speech Therapist Job Responsibilities:

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients.
• Assist as needed with patient screens for Speech Therapy.
• Communicate progress and any barriers toward SLP goals with interdisciplinary team members.
• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each speech therapy patient's specific plan of care and recommendations for follow-up.
• Maintain productivity standards as expected by the Director of Rehab or Administrator.

Education, Experience, and Licensing Requirements:

• Current Texas licensure as an SLP is required

Salary: From $55.00 per hour
