Job summary

Hometown Home Health in Jacksonville, FL is seeking a licensed Speech-Language Pathologist to deliver patient-centered care in patients' homes. You will conduct evaluations, develop treatment plans, and work with physicians and a care team to support communication and swallowing goals.

The ideal candidate holds a Master's or Doctoral degree in Speech-Language Pathology, Florida licensure, CPR certification, and at least one year of clinical experience; CCC-SLP is preferred.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology.
• Current Florida Speech-Language Pathologist license.
• Current CPR certification; online-only certification not accepted.
• At least one year of relevant clinical experience.
• ASHA CCC-SLP preferred.

Responsibilities

• Provide speech-language pathology evaluations, assessments, and treatment in patients’ homes.
• Assess patients’ communication, cognitive, and swallowing abilities.
• Develop individualized treatment plans in coordination with the patient’s physician and care team.
• Provide therapy designed to improve communication, speech, language, cognition, and swallowing as appropriate.
• Educate patients and caregivers on treatment plans, strategies, and home exercises.
• Monitor patient progress and adjust treatment plans as needed.
• Complete accurate and timely documentation, including visit notes and progress reports.
• Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team.

What We’re Searching For

Hometown Home Health is seeking a licensed Speech-Language Pathologist to provide patient-centered care in the Jacksonville area. We’re looking for someone who is comfortable working independently in a home health setting and is passionate about helping patients improve their communication and swallowing abilities.

What You’ll Do

• Provide speech-language pathology evaluations, assessments, and treatment in patients’ homes
