The Roads Home Health Care Inc. is seeking a Home Health Speech-Language Pathologist to evaluate and treat patients in their homes across South Florida. You will design individualized speech therapy plans to improve communication and swallowing function and quality of life.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology from an accredited program.
• CCC-SLP certification from ASHA.
• Current Florida SLP license and related credentials (CPR/BCLS, TB clearance).

Responsibilities

• Perform comprehensive in-home speech, language, and swallowing assessments.
• Obtain physician orders for certification and recertification of home speech therapy services.
• Deliver speech therapy treatment tailored to patient status, needs and age.
• Coordinate care planning with physicians, patients, families, and the interdisciplinary team.
• Complete timely documentation, including OASIS data sets.
• Support patients recovering from stroke or neurological conditions with individualized therapy.
• Educate patients and family on communication and swallowing strategies.

About the Role

The Roads Home Health Care is seeking a Home Health Speech-Language Pathologist (Speech Therapist) to evaluate and treat patients with speech, voice, language, cognitive-communication, and swallowing disorders in their own homes across South Florida. In this role, you'll design individualized speech therapy treatment plans that help patients regain communication and swallowing function and improve their quality of life. Both full-time and per diem opportunities are available.

• Perform comprehensive in-home speech, language, and swallowing assessments
• Obtain and communicate physician orders for certification and recertification of home speech therapy services
• Deliver speech therapy treatment appropriate to each patient's clinical status, needs, and age
• Coordinate care planning with physicians, patients, families, and the interdisciplinary care team
• Evaluate and manage patient pain using appropriate therapeutic interventions
• Educate patients and family caregivers on communication and swallowing strategies
• Complete timely, accurate documentation, including OASIS data sets
• Participate in interdisciplinary team conferences and coordinate transfer or discharge planning
• Support patients recovering from stroke, neurological conditions, or swallowing disorders with individualized treatment

Job Requirements

• Master's or doctoral degree in Speech-Language Pathology or Speech Therapy from an accredited university
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA)
• Current, valid Florida Speech-Language Pathologist license
• Minimum one year of work experience in speech-language pathology
• Current CPR/BCLS certification, current health certificate, and TB clearance
• Valid driver's license, reliable vehicle, and current auto insurance

• Excellent verbal and written communication skills for patient and family education
• Ability to work independently while managing a home visit schedule
• Compassionate, patient-centered approach to speech and swallowing therapy
• Flexible and organized, with sound clinical decision-making skills
• Genuine commitment to helping patients regain communication and swallowing function
• Prior home health experience preferred
• Bilingual candidates a plus
