Job summary

The Roads Home Health Care in South Florida seeks a Speech-Language Pathologist to evaluate and treat patients in their homes across the region. You will design individualized treatment plans to improve communication and swallowing and will work with an interdisciplinary team.

Full-time and per diem opportunities are available. Requirements include CCC-SLP from ASHA, Florida license, 1 year of experience, CPR/BCLS, TB clearance, and a valid driver's license with reliable vehicle.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology or Speech Therapy from an accredited university.
• CCC-SLP from ASHA.
• Florida Speech-Language Pathologist license (current).
• Minimum one year of work experience in speech-language pathology.
• CPR/BCLS certification, health certificate, TB clearance.
• Valid driver's license, reliable vehicle, and auto insurance.

Responsibilities

• Perform comprehensive in-home speech, language, and swallowing assessments.
• Obtain and communicate physician orders for certification and recertification of home speech therapy services.
• Deliver speech therapy treatment appropriate to each patient's status, needs, and age.
• Coordinate care planning with physicians, patients, families, and the interdisciplinary team.
• Evaluate and manage patient pain using appropriate therapeutic interventions.
• Educate patients and family caregivers on communication and swallowing strategies.
• Complete timely documentation, including OASIS data sets.
• Participate in interdisciplinary team conferences and discharge planning.
• Support patients recovering from stroke, neurological conditions, or swallowing disorders with individualized treatment.

About the Role

The Roads Home Health Care is seeking a Home Health Speech-Language Pathologist (Speech Therapist) to evaluate and treat patients with speech, voice, language, cognitive-communication, and swallowing disorders in their own homes across South Florida. In this role, you'll design individualized speech therapy treatment plans that help patients regain communication and swallowing function and improve their quality of life. Both full-time and per diem opportunities are available.

• Perform comprehensive in-home speech, language, and swallowing assessments
• Obtain and communicate physician orders for certification and recertification of home speech therapy services
• Deliver speech therapy treatment appropriate to each patient's clinical status, needs, and age
• Coordinate care planning with physicians, patients, families, and the interdisciplinary care team

• Participate in interdisciplinary team conferences and coordinate transfer or discharge planning

Job Requirements

• Master's or doctoral degree in Speech-Language Pathology or Speech Therapy from an accredited university
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA)
• Current, valid Florida Speech-Language Pathologist license
• Minimum one year of work experience in speech-language pathology
• Current CPR/BCLS certification, current health certificate, and TB clearance
• Valid driver's license, reliable vehicle, and current auto insurance
