Stamford Nursing and Rehabilitation in Stamford, TX is seeking a full-time Speech-Language Pathologist to develop and implement plans of care for residents and short-term patients. You will collaborate with an interdisciplinary team to monitor progress, address barriers, and ensure compliant documentation across regulatory and payer requirements.
The role requires current Texas SLP licensure, with a focus on maintaining a safe rehab area and providing patient/family training.
This position is for a skilled nursing facility under new ownership in Stamford. Stamford Nursing and Rehabilitation has both short-term and long-term patients. This facility has an in-house therapy team with smaller daily case loads withan emphasis on optimizing time with patients. We have flexible scheduling!

Develop and implement a Speech Therapy plan of care for facility residents and short-term patients.
Assist as needed with patient screens for Speech Therapy.
Communicate progress and any barriers toward SLP goals with interdisciplinary team members.
Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.
Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
Provide and document patient, family, and caregiver training regarding each speech therapy patient’s specific plan of care and recommendations for follow up.
Maintain productivity standards as expected by Director of Rehab or Administrator.
