Job summary

UnityPoint Health is seeking a Speech Therapist - Home Health to deliver compassionate evaluation and therapy in patients' homes. You will work with physicians and care teams to develop individualized plans of care aimed at improving communication, swallowing, and quality of life.

The position requires a Master’s in Speech-Language Pathology, state licensure as an SLP, and at least 1 year of relevant experience. A valid driver’s license and CPR certification are also required.

Qualifications

• Master’s degree in speech-language pathology.
• Licensed as a speech-language pathologist in the state(s) where services are provided.
• If the state does not require an SLP license, complete 350 clock hours of supervised practicum and 9 months of supervised full-time SLP work; pass a national examination.
• Valid driver's license with automobile insurance as required by state and/or organization.
• Mandatory Reporter: Child & Dependent Adult Abuse.
• CPR (BLS Healthcare Provider) certification.
• Minimum experience: 1 year caring for a similar patient population.

Responsibilities

• Make visits to patients' homes for home health speech therapy and conduct ongoing interdisciplinary assessments.
• Perform initial and comprehensive assessments per agency policies.
• Evaluate for dysphagia, language, cognition, and memory deficits.
• Oversee treatment modalities and communication devices as needed.
• Deliver high-quality care and maintain clear, concise clinical documentation compliant with CoPs and payer criteria.

• Makes visits to the homes of patients requiring home healthspeech therapyservicesand assumes responsibility foran ongoing interdisciplinaryassessmentof the patient.

Education:

• Master’s degree in speech-language pathology

Licensure/Certifications:

• Licensed as a speech-language pathologist by the state(s) where providing services

• If state does notrequireSLP license – completion of 350 clock hours of supervised clinical practicum; completions of 9 months of supervised full-timespeech-language pathology services after obtaining a master’s or doctoral degree; and completion of a national examination in speech-language pathologyapproved by the Secretary of Health and Human Services.

• Valid licensed driver with automobile insurancein accordance withstate(s) and/or organizational requirements.

• Mandatory Reporter: Child & Dependent Adult Abuse.

• CPR:Maintaina valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

Minimum Experience:

• One yearexperience caring forsimilarpatient population as to be assigned.

Other:

• Use of usual and customary equipment used to perform essential functions of the position.
