Job summary

Progressive Rehab is seeking a Speech-Language Pathologist (SLP) to evaluate and treat patients in Tyler, TX. The role focuses on improving communication, cognition, and swallowing through individualized therapy plans.

The SLP will collaborate with physicians, nurses, dietitians, and families, ensure regulatory compliance, and participate in ongoing professional development. A Master’s in SLP and state licensure are required, with CCC-SLP preferred.

Responsibilities

• Evaluate patients’ speech, language, cognitive-communication, and swallowing abilities using standardized assessments and clinical judgment.
• Develop individualized treatment plans based on patient needs, physician orders, and regulatory requirements.
• Implement interventions to improve speech, language, voice, fluency, cognition, and swallowing.
• Provide dysphagia management and treatment of swallowing disorders.
• Educate patients and caregivers on communication strategies, swallowing precautions, and home programs.
• Recommend diet modifications and adaptive strategies as appropriate.
• Complete all required clinical documentation in accordance with Medicare and company guidelines.
• Collaborate with interdisciplinary team members and participate in care planning meetings.

Speech-Language Pathologist (SLP) Job Description

I. Purpose and Overall Duties

The Speech-Language Pathologist (SLP) evaluates, plans, and implements individualized speech and language therapy programs for patients in a long-term care or related setting. The role focuses on improving communication, cognition, and swallowing function to enhance patient safety and quality of life.

II. Essential Job Functions

A. Patient Evaluation and Treatment Planning

• Evaluate patients’ speech, language, cognitive-communication, and swallowing abilities using standardized assessments and clinical judgment
• Develop individualized treatment plans based on patient needs, physician orders, and regulatory requirements
• Establish measurable goals and modify treatment plans as necessary based on patient progress

B. Patient Care / Intervention

• Implement therapeutic interventions to improve speech, language, voice, fluency, cognition, and swallowing function
• Provide dysphagia management, including assessment and treatment of swallowing disorders
• Educate patients and caregivers on communication strategies, swallowing precautions, and home programs
• Recommend diet modifications and adaptive strategies as appropriate

C. Documentation / Compliance / Outcomes

• Complete all required clinical documentation accurately and timely in accordance with Medicare, regulatory, and company guidelines
• Maintain appropriate records of patient evaluations, progress notes, and discharge summaries
• Ensure compliance with all federal, state, and payer regulations
• Monitor patient outcomes and adjust interventions to support optimal results

D. Collaboration / Interdisciplinary Coordination

• Collaborate with interdisciplinary team members to coordinate patient care
• Communicate effectively with physicians, nursing staff, dietitians, and families regarding patient status and recommendations
• Participate in care planning meetings and team conferences

E. Professional Development

• Maintain current licensure and clinical competencies
• Participate in continuing education and professional development activities

V. Essential Physical Demands

• Ability to sit, stand, and move between patient care areas throughout the workday
• Ability to position or assist patients as needed for evaluation or treatment activities
• Occasional bending, reaching, or positioning of materials or equipment
• Ability to demonstrate exercises or techniques related to speech and swallowing therapy
• Ability to use proper body mechanics in accordance with safety standards

VI. Work Environment

• Long‑term care or clinical setting
• Ability to travel to various locations where patients are located, including long‑term care, short‑term care, and private homes. This work will involve ability to enter and access buildings, work with patient in their existing space. Varying environmental conditions may be encountered while traveling.
• Indoor environment with occasional travel between patient areas or facilities as needed
