Job summary

THREEMEADOWS is seeking a compassionate Speech-Language Pathologist (SLP) to help older adults achieve independence and quality of life in our Northwest Ohio facility.

You will evaluate and treat residents with communication, cognitive, and swallowing disorders, coordinating with physicians, nursing staff, and families to develop individualized plans that promote positive outcomes and successful discharge.

Qualifications

  • Current Speech-Language Pathologist license in Ohio.
  • CCC-SLP preferred; Clinical Fellows may apply if supervision is available.
  • Strong clinical skills in dysphagia, cognition, and communication disorders.
  • Excellent communication, documentation, and organizational skills.
  • Team-oriented professional delivering exceptional care to older adults.

Responsibilities

  • Evaluate residents to identify speech, language, cognitive, voice, and swallowing impairments per physician orders.
  • Develop, implement, and modify individualized treatment plans to maximize communication and swallowing safety.
  • Collaborate with physicians, nursing staff, and families to coordinate care and plan discharges.
  • Educate residents and caregivers on treatment strategies and home programs.
  • Complete accurate, timely documentation meeting facility and payer requirements.
  • Supervise CFY and students per state regulations.

Speech-Language Pathologist (SLP)

Location: Northwest Ohio

Compensation: $42.00-$48.00 per hour, based on experience, qualifications, and licensure.

As our Staff Speech-Language Pathologist, you'll evaluate and treat residents with communication, cognitive, and swallowing disorders while working closely with physicians, nursing staff, therapists, residents, and families to develop individualized treatment plans that promote successful outcomes.

What You'll Do
  • Evaluate residents to identify speech, language, cognitive, voice, and swallowing impairments while determining rehabilitation potential in accordance with physician orders and facility policies.
  • Develop, implement, and modify individualized treatment plans that maximize communication, cognitive function, swallowing safety, and overall independence.
  • Collaborate with physicians, nursing staff, interdisciplinary team members, residents, and families to coordinate care, monitor progress, and plan for successful discharge or long-term maintenance.
  • Educate residents, families, and caregivers on treatment strategies, home programs, aspiration prevention, safe swallowing techniques, and cognitive support strategies.
  • Participate in interdisciplinary care conferences, rehabilitation meetings, utilization reviews, and other team meetings to ensure exceptional resident care.
  • Complete accurate, timely, and compliant documentation that meets facility, regulatory, and payer requirements.
  • Assist the nursing department by training and supporting Restorative Aides and other care team members.
  • Provide ongoing education and in-service training for facility staff to promote best practices in communication, cognition, and dysphagia management.
  • Supervise Clinical Fellows (CFY) and speech-language pathology students in accordance with state practice regulations.
  • Recommend and coordinate appropriate adaptive equipment or environmental modifications that improve resident safety, communication, cognition, and swallowing function.
  • Maintain a safe treatment environment by following infection control, safety, and equipment maintenance standards.
  • Participate in quality improvement initiatives and contribute to the continued growth and success of the rehabilitation department.
  • Build positive, professional relationships with residents, families, physicians, and fellow team members while delivering compassionate, resident-focused care.
What We're Looking For
  • Current Speech-Language Pathologist license in the State of Ohio.
  • Certificate of Clinical Competence (CCC-SLP) preferred; Clinical Fellows are encouraged to apply if supervision is available.
  • Strong clinical skills in dysphagia, cognition, and communication disorders.
  • Excellent communication, documentation, and organizational skills.
