Job summary

Storybrook Care and Rehabilitation Center is seeking a Speech Language Pathologist to provide evaluation and treatment services in a skilled nursing setting, helping residents regain quality of life. CFY with SNF experience welcomed; Colorado licensure and CCC-SLP preferred.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current state licensure or in the process of licensure in this state.
• Current basic Life Support certification.
• Must be clinically driven, dependable, and self-motivated.
• Can adjust with changes in medical field with according to CDC guidelines.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Perform patient evaluations and develop treatment plans with goals.
• Carry out patient care plans and progression within the care plan.
• Perform tests and complex treatments.
• Collaborate with therapists, physicians and healthcare team.
• Document evaluations, progress notes, and discharge summaries.
• Provide a full range of speech therapy modalities.
