The clinician will assess communication, cognitive, swallowing issues, and provide treatment with ongoing reassessment. This position may work at care sites other than the home care site.

Qualifications

• Master's Degree in Speech Pathology (Required).
• ASHA CCC-SLP certification (Required).
• Driver's License only if in Home Care department.

Responsibilities

• Direct Patient Care (90-100%).
• Evaluates and Treats Patients after provider referral and develops plan of care.
• Plans and coordinates discharge with multidisciplinary team.
