Job summary

OhioHealth at Home in partnership with Compassus seeks a Home Health Speech Language Pathologist for PRN shifts in the Columbus area. The role involves evaluating and treating patients with speech, language, cognitive and swallowing disorders within home health settings under physician direction and in collaboration with family and care teams.

The SLP will perform OASIS/Agency assessments, develop and adjust care plans, provide patient education, and ensure documentation accuracy while upholding

Qualifications

• Bachelor's degree in Speech Language Pathology is required.
• Master's degree in Speech Language Pathology is preferred.
• Minimum of one year of experience in a home health setting is required.

Responsibilities

• Provides care with infection control measures that protect staff and patient.
• Participates in in-service programs to maintain competency.
• Accepts clinical assignments within education and competence for home care.
• Monitors cases to ensure compliance with third-party payors.
• Communicates effectively with patient, family, team, physician for coordinated care.
• Maintains continuity of quality patient care with proper documentation.
• Supervises Home Health Aide/UP to ensure service quality.
• Provides speech therapy services and treatments requiring specialized skills.
• Educates client/family about the disease process.
• Performs initial and ongoing comprehensive SLP assessments (OASIS/Agency).
• Develops/revises Plan of Care with physician input.
• Performs other duties as assigned.
