OhioHealth at Home is looking for a Home Health Speech Language Pathologist - PPV PRN to provide services to clients with speech and language complications. This role involves coordinating care under physician guidance and supervising Home Health Aides.

The ideal candidate will possess at least a Bachelor's degree in Speech Language Pathology, relevant experience, and an active state license. The position emphasizes collaboration and commitment to quality patient care.

Qualifications

• Minimum of one (1) year of experience in a home health setting required.
• Active and unencumbered Speech Language Pathologist license in state(s) of employment required.

Responsibilities

• Provides services to clients with speech and language complications.
• Coordinates care under the direction of the attending physician.
• Supervises and provides clinical directions to the Home Health Aide.
• Performs assessment (OASIS/Agency) of speech, language, cognitive, and swallowing disorders.
• Develops and documents the Plan of Care.
