Job summary

Sweetwater Care in Denver, CO seeks a licensed Speech-Language Pathologist to assess and treat patients with speech, language, swallowing and cognitive disorders in a SNF setting.

You will develop treatment plans, document progress, collaborate with other therapists and physicians, and ensure compliance with Medicare/Medicaid guidelines. This is a full-time, on-site role focused on compassionate patient care and advocacy.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation.
• Provides professional treatment while using therapeutic techniques for rehabilitation of speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.
• Serves as a patient and rehabilitation advocate; participates in quality assurance and customer service activities.
• Perform patient evaluations and develop treatment plans with short- and long-term goals in line with physician orders.
• Collaborate with the healthcare team to implement and progress patient care plans.
• Document evaluations, progress notes, and discharge summaries in timely fashion.
• Ensure delivery of quality rehab services and maintain compliance with company policies and applicable laws.
