Responsibilities

1. Assessment of referred patients requiring rehabilitation services.
2. Providing direct patient care to assess medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential.
3. Establishing and administering treatment programs with goals tailored to patient capacity and tolerance, under physician guidance.
4. Setting realistic and achievable goals for patients.
5. Documenting and recording patient conditions.
6. Educating patients and families on therapy methods.
7. Preparing evaluations, weekly progress notes, recertification, and discharge documentation.
8. Collaborating with all disciplines to provide patient-centered treatment and care.
9. Performing duties as assigned by the Director of Rehabilitation.
10. Ensuring compliance with confidentiality, HIPAA, and Federal Residents' Rights.

Qualifications/Requirements

1. Graduation from an accredited Speech Therapy college or university.
2. Valid professional license.
3. Experience with long-term and/or geriatric care is preferred but not required.
