Job summary

Hill View Retirement Center in Portsmouth, OH seeks a compassionate Speech-Language Pathologist to evaluate and treat residents with speech, language, cognition, voice, and swallowing disorders.

You will collaborate with nursing, therapy, dietary, physicians, residents and families to help each resident achieve the highest possible level of independence and quality of life, within an interdisciplinary care team focused on communication and swallowing safety.

Responsibilities

• Establish measurable short-term and long-term treatment goals.
• Complete evaluations, progress notes, discharge summaries, and other documentation accurately and timely.
• Educate residents, families, and staff regarding communication strategies and swallowing precautions.

Speech-Language Pathologist (SLP)

Hill View Retirement Center is seeking a compassionate and motivated Speech-Language Pathologist to join our skilled nursing facility’s therapy department.

The SLP will evaluate and treat residents experiencing difficulties with speech, language, cognition, communication, voice, and swallowing. This position works closely with nursing, therapy, dietary, physicians, residents, and families to help each resident achieve the highest possible level of independence and quality of life.

The Speech-Language Pathologist evaluates, develops, and implements individualized treatment plans for residents experiencing speech, language, cognitive-communication, voice, and swallowing disorders. The SLP works as part of the interdisciplinary care team to help residents achieve their highest practicable level of communication, swallowing safety, independence, and quality of life.

Essential Duties and Responsibilities

• Evaluate residents for speech, language, voice, cognitive-communication, and swallowing disorders.

• Develop individualized plans of care based on assessment findings, physician orders, resident goals, and clinical needs.

• Provide skilled treatment for conditions including aphasia, dysarthria, apraxia, cognitive impairment, voice disorders, and dysphagia.

• Conduct clinical swallowing evaluations and recommend appropriate diet textures, liquid consistencies, positioning, and swallowing precautions.

• Recommend instrumental swallowing assessments when clinically appropriate.

• Monitor residents’ responses to treatment and modify interventions as needed.

• Establish measurable short-term and long-term treatment goals.

• Complete evaluations, progress notes, treatment records, discharge summaries, and other required documentation accurately and timely.

• Ensure documentation supports the medical necessity and skilled nature of the services provided.

• Educate residents, families, nursing staff, dietary personnel, and caregivers regarding communication strategies and swallowing precautions.

• Participate in care-plan meetings, interdisciplinary team meetings, utilization reviews, and discharge planning.

• Collaborate with physicians, nurses, dietitians, occupational therapists, physical therapists, social services, and other healthcare professionals.

• Identify changes in a resident’s condition and promptly communicate concerns to the appropriate clinical staff.

• Assist with restorative nursing and staff-education programs related to communication and swallowing.

• Recommend and train residents in the use of communication devices or other adaptive equipment when appropriate.

• Support residents during transitions between levels of care and upon discharge to the community.

• Follow infection-control procedures and all applicable safety precautions.

• Maintain treatment areas, supplies, and equipment in a safe and orderly condition.

• Protect resident privacy and maintain confidentiality in accordance with HIPAA requirements.

• Comply with facility policies, professional standards, payer requirements, and applicable federal and state regulations.

• Participate in quality-assurance, performance-improvement, and survey-readiness activities.

• Perform other duties consistent with the position as assigned.
