Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Silvis, IL. This role supports planning, implementing, and supervising speech-language pathology services on-site.

The position welcomes Full Time, Part Time, or PRN commitments and requires CCC eligibility, active state licensure, and ongoing professional development to maintain licensure.

Qualifications

• Master's degree in Speech Language Pathology.
• CCC certification and current licensure in state(s) of practice.
• Active SLP license in state(s) of practice.

Responsibilities

• Provide quality speech therapy services per standards and referrals.
• Evaluate and collect data for treatment planning and goals.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patients, families, and staff to ensure carryover of remediation.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and site program coordination.
• Participate in resident needs screening with clinical excellence.

Job Position

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Full Time, Part Time, or PRN

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
