Job summary

Evolve Therapy Services seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Illinois. The role supports planning, implementing, and directing speech pathology services with a focus on resident care quality.

Full-time, part-time, or PRN positions available. You will evaluate residents, develop treatment plans, instruct families and staff, and ensure CMS-compliant documentation in NetHealth EMR. Active state licensure and CCC are required.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC credential required.
• Active state licensure in practice areas.
• Ongoing CEUs to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals.
• Evaluate and obtain data for treatment planning.
• Develop treatment plans with targets and goals.
• Instruct patient, family and staff for carryover of remediation.
• Complete CMS-compliant documentation in NetHealth.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening to identify needs.

Job Position

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Full Time, Part Time, or PRN

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
