Job summary

Evolve Therapy Services, LLC seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Freeport, IL. This PRN role focuses on planning, implementing, and directing comprehensive speech pathology services to ensure high-quality resident care.

Requirements include a Master's in Speech-Language Pathology, CCC, and an active SLP license in practice states. Ongoing continuing education is expected to maintain licensure and certification.

Qualifications

• Master's degree in Speech-Language Pathology required.
• CCC/ASHA certification preferred.
• Active SLP license in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services per standards and productivity targets.
• Evaluate and collect data for treatment planning and implementation.
• Develop and implement treatment plans with short- and long-term goals.
• Instruct patient, family, and staff for carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Screen residents to identify needs and ensure timely care.

Job Purpose:

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
